When people are no longer able to care for themselves in their own homes, a move, either temporary or permanent, may be necessary. Assisted living facilities and adult family homes vary considerably in services offered. The Elder Mediation Program assists elders, their families, and caregivers with resolving conflict that may arise around issues of autonomy, housing, health care or end-of-life decisions. Trained mediators will meet with families in Mason County and throughout the region.

Ideas and insight…learning to live with loss

Bereavement is a solitary sojourn that every individual makes during a lifetime. As the years pass, we mourn the loss of parents, spouses and friends. Each loss is mourned differently, but most people manage to eventually master their pain with time and support from friends and loved ones. For others, however, bereavement continues to color all aspects of daily living. Sometimes it takes more than time and love to heal the deepest wound of all – the loss of someone much loved. The road to recovery from grief can be a long one, but the pain can be lessened when others share the journey with you.

Grief support groups and counseling can be enormously beneficial to families or individuals who experience this kind of loss. Whether it’s individual counseling or group support, the importance of getting the pain out is vital.

The majority of older people are most comfortable in their own home and want to manage by themselves, if at all possible. Following are some things you can do that will help convince others you are able to stay in your home:

1.     Eat. Don’t skip meals; if you aren’t hungry, eat a small amount anyway. Get rid of spoiled, moldy, and stale food. Occasionally invite your children over for a meal.

2.     Organize your finances – pay your bills. Budget. Be cautious when donating money – give only to charities that you are familiar with. Suppress the urge to give money to anyone with a sad story, even those on TV. Don’t give money to people who phone you or come to your door unasked. If necessary, ask for assistance when making major financial decisions.

3.     Keep your home clean and uncluttered. One of the clearest signs of dementia is living in clutter. Get rid of things you no longer need. If you need help with routine cleaning, windows, etc., hire it done. If you can’t afford that, then ask for help in finding free chore services. (Also see “Long Term Care Assistance”)

4.     Take care of your physical self. See your doctor and dentist regularly. Take your medications, brush your teeth, and exercise every day. Bathe often and wear clean clothes. Watch your weight, blood pressure and cholesterol. If you have a chronic illness, i.e., diabetes, follow your treatment plan.

5.     Stay in the mainstream. Keep in contact with neighbors, friends, clubs, children, etc. They can support you when you need help and cheer you up when you are ill.

6.     Use community services. The government and many private organizations provide services to assist you; for example, there is a bus service that will come to your door at a specified time, take you where you want to go, and bring you home when you are finished. And the good news is that you don’t have to hunt for these services, just call the Mason County Senior Activities Center at 360-426-7374.

Long-term care is a phase used to describe a wide range of services that are available to help those who find it difficult to maintain their independence. Some of the reasons for this difficulty can be related to chronic illnesses or other health-related problems, surgeries, accidents, or the gradual loss of capabilities associated with the natural aging process. There are many services available in Mason County that help people remain in or return to their own home following a medical or emotional crisis. Q:  What is the Area Agency on Aging?

A:   The Lewis-Mason-Thurston Area Agency on Aging (LMTAAA) was created in 1976 in response to the growing number of older people and the diversity of their needs. LMTAAA advocates for elders and disable adults as well as plans, organizes, develops and funds services for them.

Q:  I have been heard about something called COPES. Just what do they do for seniors?

A:   Community Options Program Entry System (COPES) is the Medicaid program designed to allow people to remain in their home and receive in-home and community-based services. Under the Medicaid program the same services may also be available in a licensed assisted living home or adult family home. Some of the services that may be available are adult day care, transportation, environmental modifications, emergency response systems, and home delivered meals. For more information, contact:

Q:  What is DSHS Home and Community Services?

A:   DSHS standards for Department of Social and Health Services. They help people gather information and provide assistance to access needed services through a comprehensive assessment process.

Q:  Is there help for learning to live with the loss of a spouse or friend?

A.   There are grief support groups – a group of people who come together to help each other through their time of grief. (See “GOOD Grief”)

Q:  I live alone and am becoming concerned for my safety if I should fall or become too ill to take care of myself.

A:   You may want to check into a personal emergency response system. These small remote control units have a button to be pressed in case of need, which will dispatch emergency people to your aid. They are available though local hospitals and companies. You may also want to contact telephone reassurance. This is a program to reassure persons living alone that they will be contacted daily. If there is no answer, action is taken to discover if an emergency exists and to notify designated people. (See “Personal Emergency Response Systems”)

Q:  There could come a time when my spouse or I can no longer make decisions for ourselves. What can we do to prepare ahead of time?

A:   There are documents you can create yourself including Power and Attorney and Durable Powers of Attorney. Many of the government agencies can give information, explanations, and advice on creating these documents as well as others documents including wills. Your senior center or community center may also have classes or speakers on the subject. Consider also seeking legal advice. Q:  What is a guardianship?

A:   If you lack legal documents specifying what you want to happen if you lose the ability to make your own decisions, someone may have to file for a guardianship. A guardianship can be expensive and can take a long time. It will be up to the appointed guardian to make decisions for you. The decisions may or may not be the ones you would make if you could. To avoid such an eventuality, the best solution is to plan ahead. Seniors 60 and older can call the Northwest Justice Project’s free statewide advice and referral line at 1-888-387-7111. Q:  What exactly does the State Ombudsman Program do?

A:   When people are no longer able to care for themselves in their own home, a move may be necessary. The Long Term Care Ombudsmen Program has trained volunteers who listen to questions and concerns and act as advocates, protecting the rights of residents in long-term care.

Q:  What is Long-Term Care?

A:   This is a phrase used to describe a wide range of services that are available to help those who find it difficult to maintain their independence. Services range from helping seniors remain in their home to finding alternate housing. (See “Long Term Care Assistance”)

Q:  What is the difference between Adult Family Homes and Assisted Living Homes and how are both of them different from Nursing Homes?

A:   Both adult family homes and assisted living provide only a minimum of nursing care and both provide basic services such as laundry, room, board, assistance with personal care such as bathing, and social services. Adult Family Homes are licensed for up to six residents. Assisted Living Facilities can be licensed for six or more residents. Individually, they differ. Some may have some medical staff; some may provide transportation for medical care, social activities, exercise programs, or a visiting nurse and/or doctor program.

Skilled Nursing Facilities are licensed, certified, and surveyed by DSHS. These facilities provide 24 hour nursing care, personal care, therapy, supervised nutrition as well as room and board. They are regulated by building and life safety codes. Some may be certified to participate in Medicare, Medicaid or Veteran Administration Programs. For information, contact DSHS at 1-800-422-3268. Q:  How can I be sure I have selected a facility for long-term care that has a good reputation?

A:   Contact the Long-Term Care Ombudsman at 360-943-6018 or 1-800-562-6028. Also, check the facility with the Better Business Bureau. Visit the facility with family or friends before deciding and talk to as many residents as you can. Most facilities will allow you to sample the food too, if you visit around the dining hour.
